If you are a private individual, you can refer yourself for an appointment using the 'Book Now' button below. This will take you to a diary page that will show you which times and days are available. When you have made a booking, you will be sent an automatic email confirmation and an invoice explaining payment options.  

If you are referring on behalf of someone else, either as a GP, health service provider or on behalf of another organisation, please complete the referral form below.
